Walk-in and after hours appointments are available, but are subject to Dr. Rehmann's availability on any given day.

On The Day Of Your Appointment:
Bring completed 8500 Form and confirmation # (see #1 link above) OR completed Basic Medical Form 8700-2 (form 8700-2 is for Basic Med. Exams only)
Corrective lenses: bring glasses/contacts being used for vision correction; if using contacts bring glasses and contact case, since vision needs to be checked with/without glasses/contacts
SODA (Statement of Demonstrated Ability): if you have a SODA have # available
Special Issuance: if you have a special issuance bring a copy of the last letter from the FAA regarding this which outlines necessary follow up for re-certification
Medications: bring an ACCURATE CURRENT list of ALL prescription drugs being taken, with name of drug, dosage, and instructions
Urine: a urine specimen is required at the time of the exam to test for diabetes/kidney disease
Cancellation:
If you find it necessary to cancel an appointment please call as soon as possible and no later than 24 hours before the scheduled appointment
